The Personalized Care Proposal will present the recommended number of hours and accommodations to keep your loved one safely at home.

Our office has standard working hours with at least one Care Coordinator on-call during nights and weekends. Over the course of this 90-minute discovery session, we determine the level of care required by your family member or loved one and ask a series of questions designed to help us find the best possible caregiver for each unique individual. Based on the results of our initial assessment, we provide a personalized care proposal recommending the ideal level of care required.

Our proprietary CarePairing system then matches your loved one with caregiver personality and skill profiles, going far beyond a basic background check to ensure his or her complete satisfaction and safety. We go the extra steps to find caregiver profiles with your preferences, including; similarity in social history, behavioral styles, cognitive ability, personality, communication style, values and humor.

Here at Griswold Home Care, we believe that every successful home care relationship depends on clear and regular communication between all parties involved. Our Care Coordinators don’t just refer a caregiver and disappear. We stay in constant contact with your loved one via a monthly call and quarterly in-person visit at no additional charge. During this interaction with your loved one, the Care Coordinator will check for a number of items including obvious changes in personality, temperament, and other home conditions at your request. We then communicate this information to authorized family members or other members of your care team via email or a phone call.

If, over the course of these calls and visits, any change in the care recipient’s well being is detected, we work with you and your family members to determine if any changes to the Personalized Care Plan is needed.

This program is ideal for those families whose loved one might be transitioning to an assisted living, nursing home, retirement community, or rehab center. We encourage families to contact us prior to entering into a senior housing option so that we can introduce a compatible caregiver in the comfort of your own home. Even if your loved one is going to a senior housing facility for temporary reasons, having a companion during this time will support their recovery and enable you to keep the same caregiver once they are discharged back to the home.
